Why it matters
Knowing who can access what is the foundation of security. Giving the right access on joining, removing the old access on a role change and closing every account on leaving reduce the risks from hijacked or forgotten accounts. Regular access reviews are also requested in audits.

Example
In one company, when a joiner form is filled in, accounts are opened automatically according to the person's role; when a leaver form is completed, every account is closed the same day and a summary is sent to the manager.

Common mistake
Working with shared accounts. Nobody knows who did what, and a leaver's access only ends when the whole team's password is changed.
