We run a two-location dental practice in Texas. We started looking for professional help to grow our organic traffic for local searches and high-margin services like dental implants. We received two different proposals, and we can't quite figure out the real difference between them.

The first proposal is from an independent SEO consultant asking for 1,600 USD a month. The scope covers strategy development, technical audit reports, keyword mapping, and weekly audits. However, he says our own web developer has to handle the technical fixes, and our dentists have to write the content or outsource it ourselves. The second quote is from a local digital agency asking for 3,400 USD a month, promising a turnkey package that includes content production, their in-house team fixing technical issues, and link building.

What exactly is an SEO consultant—do they just give advice, or do they handle hands-on execution? For a 15-person healthcare practice like ours without an in-house developer or copywriter, does working with a consultant make sense, or is an agency a must?

Short answer: An SEO consultant creates the roadmap and oversees the strategy; an agency is a full execution team that defines the strategy while also handling content production, technical development, and link building.

An SEO consultant will tell you which pages on your site aren't being crawled which search queries to target, or why your rankings dropped. They'll even prepare detailed technical specs. But if you don't have a developer to apply those fixes to your site or staff to write compliant healthcare content, those reports will just sit in a folder. A consultant is like a specialist doctor diagnosing the issue; they don't manufacture the medicine or administer the shot.

Agencies, on the other hand, have in-house developers, content writers, and link-building specialists included in their monthly fee. In your situation, since you don't have an internal team for these tasks, you'd have to pay for a freelance copywriter and hourly web developer on top of the 1,600 dollars for the consultant. That could bring your total spend very close to the 3,400 dollars the agency is quoting.

When comparing the quotes, check these 3 points: 1) Who writes the medical content at the agency, and do they have healthcare experience? 2) Will the consultant coordinate directly with your web developer and test the fixes themselves? 3) How many new content pieces and hours of technical revisions are guaranteed per month in the agency package? If you lack in-house execution bandwidth the agency model will be far more practical and results-oriented for your practice.

we made the exact same mistake but hired a super knowledgeable consultant who delivered a brilliant 60-page audit report... but since nobody at the clinic knew how to fix code or write articles, we spent 6 months just staring at reports while paying the monthly invoice. tbh if you dont have implementation bandwidth, a consultant is a waste of money.

In the US market, a freelance medical content writer typically costs around 0.10 - 0.20 USD per word. If the consultant recommends 6 articles a month, you'll be paying the writer 600-900 USD. Add at least 400 USD for 5 hours of developer time. The consultant's actual cost easily creeps up toward the 2,800 USD mark.

Don't blindly trust agencies promising they do everything, either. If they don't specialize in healthcare, they might have AI or inexperienced interns write about complex topics like implants, putting your site at risk with search engines. If you go with an agency, always demand a healthcare portfolio.

The consultant model works best for companies that have an in-house marketing specialist or a manager with an agency background. That manager takes the strategy from the consultant and delegates the tasks to the internal team. If you're a business owner seeing patients at your clinic while also trying to track tasks, going with an agency and dealing with a single point of contact makes much more sense.

a consultant will only tell u what to do, they wont actually do the work. tbh if u dont have anyone in-house to update your site, dont even think about hiring a consultant, youll just be burning your money.

When reviewing an agency's proposal, make sure to include the following in the contract: 1) Monthly word count deliverables and the approval process, 2) The quality and industry relevance of acquired backlinks, 3) A commitment that any technical code changes to the site will be performed with backups in place.

Here's a simple formula to help you decide: Can you dedicate at least 4-5 hours a week to this project and to managing external freelancers? If the answer is no, go with the agency model without a second thought, and consolidate all the responsibility onto a single invoice.

What's the tech stack of your clinic's website? If you're using standard WordPress or a common CMS, you can handle some of the technical tasks requested by the consultant yourself using simple plugins. But if it's custom-coded, agency support is an absolute must.

The cheap-looking path usually ends up costing more later. If code ownership isn't in the contract, you have no bargaining power when leaving.

The cheapest quote is usually the least thought-out one. This is my opinion I'm not claiming it's absolute truth.
