We run a physical therapy and clinical pilates studio in Kadıköy with 3 physiotherapists. For years, our receptionist has been managing all client appointments manually via phone calls and WhatsApp, jotting them down in a paper planner.

The digital agency we hired to redesign our website told us, "We definitely need to integrate an online booking system for you it'll completely eliminate phone traffic," and quoted an extra 55,000 TL for custom software. The idea of clients booking their own appointments online sounds convenient but we can't quite grasp what it actually entails or how it would change our daily operations.

For a clinic of our scale seeing 15–20 clients a day, what does an online booking system actually do and is it really worth the cost?

Short answer: An online booking system is a scheduling platform that lets clients view real-time open slots on your website, book, cancel, or reschedule appointments, and supports the process with automated SMS or email reminders. Its true value goes beyond reducing phone calls; it drastically cuts no-show rates and captures bookings outside your normal working hours.

However, spending 55,000 TL on custom software for a clinic with 3 physiotherapists seeing 15–20 clients daily is a massive waste of money. Today, there are dozens of ready-made, cloud-based booking platforms tailored to your industry that cover every single need. These off-the-shelf tools cost around 600 TL to 1,500 TL per month, support staff-specific calendars, handle varying session lengths, and sync two-way with Google Calendar.

Absolutely decline the agency's proposal to build something from scratch. Custom software will leave you vendor-locked to that agency every time you want to tweak an SMS template or add a new physiotherapist. Instead, have them embed a link or widget to a turnkey booking platform that will cost you around 800–1,000 TL a month. That way you protect your budget and free up at least 2 hours of phone duty for your receptionist every day.

Full automation can backfire in healthcare-focused fields like physical therapy. Letting someone pick a random time slot without understanding their condition or checking if they have a doctor's referral can wreck your schedule. Personally, I think an initial screening call is essential for first-time visits.

Don't drop 55k right away. Sign up for a free trial on one of the popular off-the-shelf scheduling platforms online. Have your receptionist enter a portion of your existing clients and test it for 15 days. You'll see firsthand whether clients can manage booking on their own.

the agency is just trying to reinvent the wheel and bill you for it. linking an existing tool to your site with a booking button takes 30 minutes. do not pay 55k for that.

A solid booking system generally works like this: 1) The client picks the specialist and service type on your site, 2) The calendar shows only available openings for them to choose from, 3) An instant confirmation SMS hits their phone, 4) 24 hours prior the system sends an automated reminder asking for confirmation.

What helped us most was capturing after-hours bookings. A client whose back flares up at 11:00 PM searching online can't call you then, but with the booking tool live they snag a spot for the next day immediately. like a surprising number of clients come in that way.

The system you choose must have these three features: 1) Individual calendars and time-off tracking per physical therapist, 2) Automated SMS and WhatsApp reminders, 3) The ability to set a policy requiring at least 12 hours' notice for appointment cancellations.

If code ownership isn't in the contract, you have no bargaining power when leaving. If I were you I'd go this route.

How did you solve this? Everyone rushing into what is an online booking system gets stuck at the same point.

The harder it is to reverse a decision, the slower you should make it. If I were you, I'd go this route.

correct in theory but it doesnt work that way in practice. payment schedules should be tied to projetc phases, not calendar dates.
