We run a physiotherapy and reformer pilates center with three branches in Bursa. We have a total of 14 instructors and physiotherapists on our team. tbh up until now we’ve tried managing appointments using branch-based Excel sheets and WhatsApp groups, but duplicate bookings, mixed-up make-up sessions and commission payout calculations have started bringing operations to a halt.

We're looking into digitizing the setup. Two paths have emerged: First, off-the-shelf cloud booking platforms with subscription fees between 1,500 and 2,500 TL per month per branch. tbh second, handing all our workflows over to a software agency to build a custom booking system from scratch for somewhere between 130,000 and 180,000 TL.

Pre-built platforms seem super practical in terms of quick setup and initial outlay. honestly but where does the real difference kick in when balancing custom software's promised flexibility and data ownership against the 3 to 5-year total cost of ownership? At what point does it actually make sense to go the custom route?

Short answer: If you have a standard session or class workflow, an off-the-shelf platform makes far more sense for speed and upfront cost. Custom development only becomes worthwhile if you have complex commission formulas unique to your clinic, need integration with an existing accounting tool, and must host data on your own servers.

The first major difference is operational fit. Ready-made SaaS platforms are built around common workflows shared by thousands of businesses. Scheduling sessions, instructor calendars, and client cancellations are all standardized. But if you have rules like freezing lesson packages, transferring quota between branches, or paying different commission tiers to different instructors, off-the-shelf platforms might fall short, forcing you to warp your processes to match their templates.

The second critical factor is data ownership. With off-the-shelf platforms, the database lives on their servers. If you cancel your subscription down the road, you usually only get to export a client contact list; past booking logs and detailed financial transactions stay behind. With custom software, both the database and the codebase live on a server you fully control.

The third difference is the long-term cost structure. Paying an average of 6,000 TL monthly for three branches on a SaaS tool runs up to 216,000 TL over 3 years, and you don't own the system. With custom software, you pay 150,000 TL upfront, but you must budget at least 20 percent of that build cost annually for hosting, security patches, and maintenance. Without an in-house tech person or a dedicated maintenance budget, custom software quickly turns into an obsolete brick.

Going down the custom software rabbit hole for a team of 14 is a waste of resources. Most small businesses that build custom software end up returning to SaaS platforms two years later because they can't find a developer to maintain it. If a ready-made platform covers 85% of what you need, absolutely start with that.

In early 2022, we spent 95,000 TL having a custom booking tool built for our clinic. Within two years, we had to pour in another 80,000 TL for server costs, mobile responsiveness patches, and regulatory updates. Last month we switched to a ready-made platform paying 3,200 TL a month; it has way more features and zero maintenance headaches.

If you go with an off-the-shelf platform, make sure to check whether it offers a REST API. Later on, when you want to connect it to your accounting software or website, a closed system with no external data access will leave you trapped. Cloud tools with webhooks and API endpoints give you flexibility that's pretty close to custom software.

Narrow your decision down to these 3 points: 1) Are your instructor payouts and make-up session logic completely outside industry standards? 2) Do you have an internal policy preventing you from storing client health history in a third-party cloud? 3) Can you commit at least 30,000 TL a year for maintenance after launch? If the answer to these is no, an off-the-shelf system is the only way to go.

the three of us partners argued back and forth for ages about getting a custom build. then we just signed up for a 14-day trial on a ready-made tool. honestly the instructors struggled a bit during the first week but by week two everything ran smoothly then instead of sinking 150k into software we spent it on branch equipment, which was a much smarter move.

Correction: I misremembered the figure, it was a bit lower.

Don't be fooled by the low first-year price of ready-made platforms. Once your entire operation relies on their tool, you have no leverage when they hit you with a 100% price hike. Migrating the full appointment history of three branches to another tool turns into a months-long nightmare.

if you build custom software and the dev who wrote it leaves nobody wants to touch the codebase they all say "let's rebuild it from scratch". I mean with an off-the-shelf system, at least you're dealing with an actual company.

Test the two most popular off-the-shelf booking tools using one-month free trials in just one of your branches. Committing to a 150,000 TL build without seeing the operational bottlenecks in a live environment is flying completely blind.

When making a decision, first look at what data you have on hand. Good luck with that.
